At Assertive Aspirations Behavioral Health Services LLC, our initial assessment and reassessment process is designed to ensure a clear understanding of each individual’s unique needs and provide the foundation for their personalized care plan. The initial assessment includes a series of integrated diagnostic, clinical, and psychosocial evaluations conducted with the recipient and their significant others. This helps us create an effective, comprehensive individualized services and recovery plan (Treatment Plan), as well as assess the recipient’s level of need and medical necessity.

The initial assessment must be completed and submitted to MBHS within 30 days of the eligibility screening date, which is noted on the ARF for SAA to begin the authorization process.

Reassessment Process: If the recipient continues to meet medical necessity criteria and services are requested, a reassessment is necessary. This includes:

  • Conducting a reassessment, rating the LOCUS or CALOCUS, and entering the rating into Clinical Magellan before submitting the authorization request.
  • Updating the Treatment Plan and ensuring the review, signing, and dating of the e-CDI printout by the LMHP and Psychiatrist/M.D.
  • Completing an Authorization Request Form (ARF) and submitting a data file with all required sections and signatures.

The reassessment must be based on any changes since the most recent authorization, including a face-to-face interview with the recipient. If the recipient is a minor, information must be obtained from a parent, legal guardian, or another person legally authorized to consent to medical care.

This data serves as the foundation for developing the recipient’s updated Treatment Plan, ensuring that care remains aligned with their evolving needs.
